And the Winner is…
Wandsworth Care Home In-Reach Team.
We are very pleased to report that the CAHS Care Home team were awarded the “Preventing Harm” award at CLCH’s annual awards ceremony on the 8th November. This award is given to the individual or team who has made an outstanding contribution to reducing unwarranted variations in care and increasing diligence in practice. The Care Home Team were nominated for “the delivery of a high standard of care to residents living in 9 local Care Homes, within the current scope of the project and for addressing the inequality of care, ensuring access to the right services to review their needs. The team help staff to develop their skills and confidence to manage residents, to identify and signpost services available in the community and reduce avoidable emergency admissions”
They collected their award from Charlie Sheldon, CLCH’s Chief Nurse.
The Care Home In Reach Team comprises of 1.8 wte Advanced Nurse Practitioners and 1 wte Senior Nurse who leads on training and education in the homes. The service has since increased its cover, and is currently in 13 homes in Wandsworth, it offers a rapid response function: homes can call if they are concerned about a patient and nurses will visit within 2 hours of the call; a routine response : responding to routine referrals within 3 days; and an education function comprising core and unplanned training.
Since the start of the project in February 2018, 517 visits have been made to residents in Care Homes in Wandsworth, with 104 preventions of transfer to hospital made as a result.
We know that this team is well respected by the GPs and wider health partners in Wandsworth, and it was great to see them recognised within their own organisation too.
